FinalForms is Holy Name's online forms and data management service. To begin registering your student at Holy Name, you must create an account as soon as possible.
Please register at https://holynameparmaheights-oh.finalforms.com/ and follow the prompts to create your account, create your student's account, and sign your forms.
We are asking that the first 3 form sections be completed by Wednesday, February 23. These sections are: Contact Information, Demographic Information, and Additional New Student Information. In order to register for classes at your student's guidance counselor appointment, these must be completed.

*Note: If you have a current Holy Name student and are already using FinalForms, please do not create a new account. You will only need to 'add' your new student to an existing account.
**Note: Your student will receive a student account in August at Freshmen Orientation. Please do not worry about entering a student email at this time.
To hold your seat in the Class of 2026, please pay the $200 non-refundable registration fee by Friday, April 8th, 2022.

The next step in the enrollment process is to meet with your student's guidance counselor, who will lead you through all aspects of Freshman Registration. During the individual appointment, you will register for your freshman courses and discuss your transition into high school. Remember, you will have the same counselor for the next four years. They are very excited to meet you and your family!
More information regarding the appointment will be covered at the Welcome Meeting on Thursday, February 24th.

If your student took a high school accredited world language class everyday during their 8th grade year, Holy Name will transcript the class and place your student appropriately. To confirm your child's placement, they may be asked to take a foreign language placement exam.
If your student took world language on a weekly basis in grade school and would like to place into a 2nd year world language class at Holy Name, they must take a world language placement exam.
All world language placement exams will be administered after your family's registration appointment with their counselor.
If your student has yet to take the High School Placement Test (HSPT), they must register to take a make-up exam on Thursday, March 31st at 5:00pm. All accepted students are required to have a test on file.

Holy Name High School accepts the Cleveland Scholarship Program (CSP). Provided through the Ohio Department of Education, this Program gives students living within the boundaries of the Cleveland Metropolitan School District the opportunity to attend private schools at a discounted rate. The high schools chosen for attendance must be located in Cleveland, or located in a district adjacent to Cleveland. Holy Name qualifies as it is located in the Parma City School District, which is adjacent to Cleveland. For high school students, the maximum scholarship amount awarded through the Cleveland Scholarship Program is $7,500. Families are then responsible for paying the remainder of the tuition balance.
If you currently are a Cleveland Scholarship Program recipient, you must fill out the Renewal Form. After filling out this form, email the completed form, as well as a copy of a current utility bill (no more than two months old and with a service address displayed) and a copy of your student’s birth certificate, to statescholarships@holynamehs.com.
If you live in the city of Cleveland, but are not currently utilizing the Cleveland Scholarship Program, we encourage you to apply. You must fill out the Request Form. After filling out this form, email the completed form, as well as a copy of a current utility bill (acceptable forms to prove residency can be found here; keep in mind it should be no more than two months old and with a service address displayed) and a copy of your student’s birth certificate, to statescholarships@holynamehs.com.

There are two EdChoice Scholarships: Traditional and Expansion. Please note that these scholarships are controlled by the state. Holy Name is here to submit your application but is not involved in the selection or distribution of the funds.
Students who are assigned to certain public high schools are eligible to apply for a $7,500 EdChoice Traditional Scholarship. The 2022-2023 list can be found here: 2022-2023 EdChoice Traditional Designated Public Schools List.
Please note that any current Holy Name student in grades 9-11 who received the EdChoice Traditional Scholarship in a previous year (based on district of residence, i.e. Parma), is eligible to renew the EdChoice Traditional Scholarship in the following year.
To renew an EdChoice Traditional Scholarship, submit to the Holy Name Main Office a completed EdChoice Renewal Form and a current utility bill (acceptable bills to submit can be found here and must show both the billing and service address, while being no more than two months old). You may also submit by emailing the paperwork to statescholarships@holynamehs.com.
To request an EdChoice Traditional Scholarship, submit to the Holy Name Main Office a completed EdChoice Request Form, a current utility bill (acceptable bills to submit can be found here and must show both the billing and service address, while being no more than two months old), and a copy of your student's birth certificate (make sure the student’s name on their birth certificate matches the name written on the application). You may also submit by emailing the paperwork to statescholarships@holynamehs.com.
Additionally, if you believe you would qualify for EdChoice Traditional – Low Income, please fill out the Income Verification Form, which will need to be submitted to the Ohio Department of Education. Traditional EdChoice Scholarship applicants qualify for low income status if income is at or below 200% of the Federal Poverty Guidelines. We strongly encourage you to fill the Income Verification Form out using the online portal. To learn how to access the Scholarship Income Verification System, view this Quick Guide for support.

If you are not able to upload your information online, you are still able to mail the Low Income Verification Form to the Ohio Department of Education. However, we do not recommend this option and encourage you to use the online portal if at all possible.
Families who qualify for EdChoice Traditional must pay the remaining tuition balance after the $7,500 scholarship is applied. However, families who have their income verified and qualify as EdChoice Traditional – Low Income do not have to pay the remaining tuition balance. Other HN associated fees will need to be paid.
EdChoice Expansion is another possibility for Holy Name students who do not live in the city of Cleveland, and who are not assigned to a public high school designated on the EdChoice Traditional List. EdChoice Expansion is based on gross family income.
Families are eligible to apply for the EdChoice – Expansion Scholarship if the family household income is at or below 250 percent of the Federal Poverty Guidelines. Here are the EdChoice Expansion Income Guidelines.
Those families whose household gross income is between 201-250 percent of the Federal Poverty Guidelines are eligible to receive a $7,500 EdChoice Scholarship, but must pay the remaining tuition balance. EdChoice Expansion recipients that have a household gross income of 200 percent or lower qualify for low-income, and Holy Name cannot charge the remaining tuition balance (other fees are required to be paid to Holy Name).
To renew an EdChoice Expansion Scholarship, submit to the Holy Name Main Office a completed EdChoice Renewal Form and a current utility bill (acceptable bills to submit can be found here and must show both the billing and service address, and must have been issued in the last two months). You may also submit by emailing the paperwork to statescholarships@holynamehs.com.
EdChoice Expansion renewal recipients do not have to pay the remaining tuition balance after the $7,500 scholarship is applied, as long as the family income is once again at or below 200 percent of the Federal Poverty Guidelines. If your income status has changed since last year, the EdChoice Scholarship award and tuition obligation amount may change.
To request an EdChoice Expansion Scholarship, submit to the Holy Name Main Office a completed EdChoice Request Form, a current utility bill (acceptable bills to submit can be found here and must show both the billing and service address, and must have been issued in the last two months), and a copy of your student's birth certificate (make sure the student’s name on their birth certificate matches the name written on the application). You may also submit by emailing the paperwork to statescholarships@holynamehs.com.
All who submit EdChoice Expansion Renewals and Requests must also fill out the Income Verification Form, which will need to be submitted to the Ohio Department of Education.
