Application Documents

Diocesan Application - Students applying for the Class of 2029 from a Diocesan Catholic grade school will receive their application electronically directly from their grade school on Thursday, January 2nd, 2025. Students will be submit their completed application back to their grade school no later than Friday, January 10th, 2025.

Non-Diocesan Application - Students applying for the Class of 2029 from public or Non-Diocesan private grade schools should submit the below documents (Non-Diocesan Application, 2 Recommendation Forms, Authorization to Release School Records) by Monday, January 20th, 2025. If you miss the deadline, please reach out directly to Holy Name Admissions by email or 440-886-0300 ext. 100.

Transfers - Holy Name will consider transfer applications for students in 9th-11th grade at the beginning of each school year and at the semester.

The Holy Name Admissions Team will begin working with transfer students in April of 2025.
Holy Name will begin making official admission decisions for transfer students starting on May 1st, 2025.
The deadline to enroll as a transfer student for the 25-26 school year is Friday, August 1st, 2025. 

In order to apply, we will need the following which can be downloaded at the bottom of the page:

• Transfer Application
• 2 Letters of Recommendation
• Signed Authorization to Release School Records
• Scheduled interview with our principal (must bring a final report card and updated transcript to the meeting)

As we take pride in our academic standards and the good character of our students, final acceptance to Holy Name is contingent upon successful completion of your year/semester.
